Job description
Sentara Home Care in Charlottesville, VA is seeking a Home Health Coordinator- Registered Nurse to join our day shift team. The role coordinates and supervises nursing care for participants, conducts initial and periodic assessments, and collaborates with an interdisciplinary team to support patient health goals.
The ideal candidate holds an Associate's Degree in Nursing, an active VA RN license, BLS within 90 days, and at least 1 year of nursing experience with preference for home health
