Home health provides the opportunity to work one-on-one with patients in their homes, helping them maintain or improve their health and manage their care.
Responsibilities
Manage and coordinate care for an assigned caseload of patients
Provide skilled nursing care using evidence-based practice and critical thinking
Collaborate with interdisciplinary team members, patients, and families to promote continuity of care
Supervise and direct support staff in the provision of care
Deliver safe, age-appropriate nursing care focused on optimal patient outcomes
