Location: San Jose Northeast and Southeast
Scheduled: Mon- Friday with on call weekends rotation
Responsibilities
Assess patient needs and develop individualized care plans to ensure optimal outcomes
Administer medications and treatments as prescribed by physicians and monitor patient responses
Collaborate with interdisciplinary teams to coordinate care and services for patients
Educate patients and families on disease management, treatment options, and self-care techniques
Document patient information and care activities accurately and efficiently in electronic medical records.
Qualifications
Current licensure as a Registered Nurse in the state of California
Minimum of 1 year of experience in home health or hospice
Strong assessment and critical thinking skills to identify patient needs and prioritize care
Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams
Ability to work in a fast-paced environment and adapt to changing priorities and situations
Proficiency in electronic medical records and other healthcare technology systems
Commitment to delivering high-quality patient care and promoting a culture of excellence and compassion

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $100 to $170 visit commensurate with experience and depending on type of visit.
