Your Role
We’re seeking a compassionate, experienced Home Health Registered Nurse (RN) to join our high-performing team. The ideal candidate has strong clinical judgement, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of skilled nursing care in the home setting.
What You’ll Do
Conduct OASIS assessments and complete state-of-care, recertification, resumption-of-care, and discharge visits
Develop and implement individualized plans of care in collaboration with physicians and the interdisciplinary team
Perform comprehensive nursing assessments and ongoing patient evaluations
Administer medications, perform skilled nursing procedures, and provide wound care as ordered
Educate patients and caregivers on disease management, medications, safety precautions, and infection prevention
Supervise and coordinate care provided by LPNs, HHAs, and other team members as appropriate
Monitor patient progress, update care plans, and maintain thorough, timely, and compliant documentation
Ensure compliance with all Medicare, CMS, and home health regulations
What We’re Looking For
Current Florida Registered Nurse (RN) license (required)
Previous home health experience is strongly preferred but not required
OASIS experience preferred
Excellent assessment, communication, time management, and documentation skills
