Job description
Job Title: RN Home Health
Address: Lake Charles, LA
Job Description
Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
Gather data on factors influencing patient/family health status and incorporate into the POC.
Maintain accurate and up-to-date patient records, documenting all visits, contacts, problems, plans, and goals.
Provide care using infection control measures to protect staff and patients.
Educate patients and families on care, medication, diet, and safety, utilizing appropriate teaching methods.
Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality service.
Critical Information
Schedule: 30 units on call required
Remote/hybrid work based on schedule
On-call responsibilities included
Environment involves home care settings and compliance with health and safety standards
Education/Licenses Needed
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
Bachelor of Science in Nursing (preferred)
Current state license as a Registered Nurse
Proof of current CPR certification
CHPN certification (preferred)
