Job Description
Assess home health and hospice patients, identify physical, psychosocial, and environmental needs. Implement, develop, and evaluate the Plan of Care to ensure quality, continuity, and patient safety. Gather data on patient factors and incorporate findings into the Plan of Care. Maintain accurate patient records, documenting visits, contacts, problems, and care plans. Provide care using infection control measures to ensure staff and patient safety. Educate patients and families regarding care, medications, and home safety practices. Supervise and direct home health aides, LVNs, and STNAs for consistent quality service.
Critical Information
Remote/hybrid work environment. On-call responsibilities required. Schedule involves approximately 30 units per week with flexible hours. Compliance with industry standards and safety protocols.
Education/Licenses Needed
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program. Current state license as a Registered Nurse. CPR certification required. Two or more years of home health nursing experience preferred. CHPN certification preferred. Reliable transportation and valid driver's license required.
