Job description
Start: ASAP
Schedule: Monday–Friday
Hours: 40 hrs/week
Productivity requirement: 30 points/week
Pay
$54 – $56/hr
Weekly Pay: $2,160 – $2,240/week
Required Credentials
Active Idaho RN License required
BLS Certification required
Home Health RN experience required
OASIS documentation experience required
Homecare Homebase (HCHB) experience required
Ability to meet productivity targets (30 points/week)
Valid driver’s license and reliable transportation required (home visits)
Responsibilities
Provide skilled nursing care in patient homes
Complete OASIS assessments and documentation
Develop and implement patient care plans
Coordinate care with physicians and care teams
Educate patients and families on care plans
Manage caseload and meet productivity requirements
Provide hospice and home health support services
