About This Role
As a Home Health Registered Nurse at Cleveland Clinic, you will provide high-quality nursing care to patients in their homes, focusing on assessments, complex treatments, and education to prevent hospital readmissions.

$69,680 - $111,488 annually

Facility: Cleveland Clinic Fairview Hospital
Schedule: Full-time, Day shift
What You’ll Do
Provide direct nursing care to patients in their homes.
Perform accurate and complete admission assessments and reassessments.
Assess patients for safety risks, abuse, and neglect.
Serve as a primary coordinator of all disciplines for patient care.
Assess, document, prioritize, and manage patients’ pain.
Complete documentation and make calls after patient visits.
Attend meetings and coordinate patient care.
Arrange visit schedules for patients.
Requirements
Education:
Graduate from an accredited school of professional nursing
Bachelor of Science in Nursing (BSN) preferred
Experience:
Minimum one year of current clinical experience as a registered nurse
One year of clinical experience as an RN in an acute med/surg setting
Licenses & Certifications:
Current RN License in Ohio
Basic Life Support (BLS) certification required
Valid Ohio driver’s license and proof of automobile insurance coverage
