Job description
Heartland Hospice, part of Gentiva, is seeking an experienced Hospice Clinical Liaison to manage intake, referrals, and admissions while building strong relationships with hospitals, physicians, and discharge planners. You will visit hospitals and facilities daily, educate families about hospice services, ensure compliance, and coordinate with the care team to deliver compassionate, comprehensive care.
The ideal candidate holds an active RN license, CPR, and 3–5 years hospice experience; BSN
