Job description
Gentiva is seeking a dedicated Patient Care Manager to lead hospice clinical operations, ensuring delivery of high-quality, compliant services. This role reports to the Executive Director or Administrator and focuses on coordinating care and staff development to meet regulatory standards.
The successful candidate will oversee scheduling, triage referrals, lead IDG meetings, and mentor team members while ensuring adherence to federal, state, and local regulations.
