A local hospice provider in Texas is seeking an RN Case Manager to join their growing team. The RN will conduct home visits for new patient hospice admissions, perform assessments, and collaborate with patient families and facilities. While hospice experience is preferred, it is not required. Candidates must have reliable transportation.
