RN Hospice & Home Health Intake Liaison

Job description

Mercy South Hospice RN – Intake in St. Louis is a PRN role handling referrals for Hospice, Home Health and Home Infusion across acute care and clinic settings. You will liaise with discharge planners, social workers, physicians and prospective patients to facilitate timely discharges and ensure continuity of care within Mercy Health System.

Requires an active RN license and 3+ years nursing experience; Hospice/Home Health experience preferred. CHPN certification a plus.
