Job description
Activate Care in Massachusetts is seeking a Registered Nurse to perform in-home assessments for dually eligible members under a managed care health plan. The RN will travel to members' homes, complete timely assessments, document findings, and hand off to care management.
The role requires MA licensure, 1 year home health experience, driving, and HIPAA compliance. Flexible hours, potential sign-on bonus, and full-time benefits are offered.
