Job description
FreedomCare seeks a Registered Nurse (RN) for Home Health Case Management in Connecticut. The role includes initial in-home assessments, care coordination, and ongoing support for patients and caregivers. You will conduct regular visits, develop plans of care, and work with interdisciplinary teams to ensure high-quality, person-centered care.
New graduates are welcome. Travel within the assigned geographic area is required; a valid CT RN license and car insurance are essential.
