Job description
Sentara Obici Hospital in Suffolk, VA is hiring an Inpatient Case Manager (RN) to coordinate comprehensive care plans and oversee patient transitions to appropriate levels of care. The role emphasizes collaboration with the care team and patient/caregiver to ensure quality outcomes and efficient LOS.
The ideal candidate holds an RN license for Virginia (or multi-state), BLS certification, and at least 1 year of nursing, with acute care and case management experience preferred.
