Job description
CareSource Commonwealth Care Alliance seeks an Integrated Care Clinical Manager (RN) in Massachusetts. This field-based role delivers complex care management for dually-eligible enrollees, coordinating with primary care, specialists, and community resources to improve health outcomes and quality of life.
You will lead the ICT to develop holistic care plans, educate members on benefits, and ensure seamless care transitions.
