RN/LMSW Case Manager: Hospital Care Navigator

Job description

Cayuga Health, A Member of Centralus Health, is looking for an RN/LMSW Case Manager responsible for delivering comprehensive care to assigned patients. Key responsibilities include assessing needs, developing appropriate Plans of Care, and guiding patient progression across the care continuum.

The ideal candidate should possess a degree from an accredited nursing or social work program, have 2-3 years' experience in an acute care setting, and have strong communication and problem-solving skills. This role is onsite based in Ithaca, New York.
