Job description
Careforth, Inc. seeks an RN Care Manager or LSW Care Manager to onboard new families and provide ongoing caregiver support. You will assess consumer and caregiver needs, determine eligibility, and guide activation of services with a person-centered plan of care.
Regular home visits and coaching are expected. The role requires mentoring caregivers on best practices, collaborating with the clinical team, and ensuring regulatory compliance.
