Job description
Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking a talented MDS Coordinator with RN licensure to join our team. The role focuses on accurate MDS completion, CAAs, and care plans in line with federal and state rules while coordinating services with physicians, families, and payers.
The coordinator acts as in-house case manager, collaborating with the IDT to develop care pathways, assess RUG scores, and monitor regulatory risk and outcomes for residents.
