Job description
Ohana Pacific Health in Hale Kupuna, Koloa, HI is seeking a Full-time Minimum Data Set (MDS) Coordinator / RN to ensure timely, accurate MDS assessments, care plans, and electronic transmission for residents.
The role requires coordinating the Interdisciplinary Team (IDT) in the RAI process, reviewing MDS/Care Plans, and ensuring sign-off for completeness. Knowledge of RAC-CT and state regulations is essential.
