Job description
Livingston Hills Rehabilitation & Nursing Care Center in New York is seeking an experienced RN MDS Coordinator to join our team. You will ensure timely and accurate assessments of interdisciplinary care plans and identify resident needs while communicating care expectations to families.
You will use assessment data to help formulate care plans and monitor their effectiveness in coordination with social services, dietitians, rehab specialists and medical staff.
