Livingston Hills Rehabilitation & NCC is seeking an experienced RN MDS Coordinator to join our team in Columbia County, NY. This 120-bed SNF offers weekly pay, a strong benefits package, health insurance, generous PTO, and opportunities for professional growth.
Responsibilities include timely assessments of interdisciplinary care plans and ensuring compliance, with collaboration from social services, dietitians, rehab specialists and medical staff. NY RN licensure and MDS experience required.
