About This Role
The RN Navigator - Gastroenterology at Kaiser Permanente provides professional nursing care focused on patient education and resource facilitation. This role is ideal for nurses who excel in patient advocacy and care coordination throughout the healthcare continuum.

What You'll Do
Assist patients and families in understanding their diagnosis and treatment options.
Conduct assessments, symptom management, and case management for patients with chronic diagnoses.
Develop and facilitate individualized care plans in consultation with healthcare providers and family members.
Serve as a link between patients, healthcare providers, and support teams.
Facilitate changes in care plans and broker necessary services.
Perform care assessments and triage of patients via phone or in-person.
Facilitate appointments for consults, procedures, and support services.
Standardize patient education materials and maintain a resource library.
Deliver patient and family education throughout the treatment process.
Monitor patient compliance with treatment plans and assist with referrals.
Requirements
Education
RN License in Georgia required
Experience
2+ years in a nursing role preferred
Licenses & Certifications
Current RN License in Georgia
BLS certification required
