Job description
Premier Genesee, a leading senior care provider in Batavia, NY, is seeking a dedicated LPN/RN Nurse Case Manager to coordinate patient care and services, ensuring residents receive high-quality treatment and support. The role involves assessing needs, developing care plans, coordinating with providers and families, monitoring progress, educating patients and caregivers, and participating in quality initiatives.
On-site schedule Monday–Friday 8am–4:30pm with on-call every other week.
