Job description
Prosser Memorial Health is seeking a Nurse Navigator who is a professional RN to offer individualized assistance across the care continuum. The role focuses on high-risk patients, ensuring coordinated care and overcoming health system barriers.
The ideal candidate should possess excellent communication skills, RN licensure, and proficiency in Microsoft Office. Preference will be given to those with experience in nurse navigation and national training.
The position offers a competitive pay range of $41.63-$77.01 per hour.
