n nWhat You'll Do
nnProvide direct patient care in the home
nMonitor patient condition and report changes
nAdminister medications and treatments as ordered
nSupport individualized care plans
nDocument care and communicate with the healthcare team
n nAdditional Details
nnAdult and pediatric patient opportunities available
nHome-based care within an assigned service area
nOrientation, onboarding, and ongoing clinical support provided

nRequired:
nnActive LPN/RN license
nCurrent BLS certification
n nPreferred:
nn1+ year of nursing experience
