nnDedicated internal after-hours triage team to support nurses outside of regular business hours

nnHomecare Homebase (HCHB) EMR for streamlined documentation
n
n nOffice Location: 1028 S. Main Street Suite C Fond du Lac, WI 54935
nCoverage area: Travel throughout the territory is required - Serving Winnebago & Fond du Lac Counties
nSchedule: Monday-Friday 8AM-5PM

nResponsibilities
nAs a RN Case Manager You will:
nnnAssess/ monitor physical, emotional, and psychological needs of patients
n
nnCreate hospice care plans that align with the patient's wishes and goals
n
nnDirect nursing care: administering medications, treatments, and interventions
n
nnProvide pain and symptom management
n
nnEducate and support the patient’s family and caregivers
n
nnCollaborate with interdisciplinary team
n
nnMaintain accurate and timely documentation
n
nnParticipate in on-call rotation as required by local branch
n
n nQualifications
nnnnGraduate of an accredited nursing school with current licensure to practice in state of operation
n n
nnOne year nursing experience required , 3+ years preferred
n
nnExperience in hospice or a similar setting preferred
n
nnKnowledge of the hospice philosophy of care
n
nnCommitment to clinical and documentation excellence
n
nnMust be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
n
