n nnAs a Hospice Admissions RN You will:
nnnVisit patients (in homes, hospitals, nursing facilities, etc.) who are referred for h ospice care
n
nnExplain hospice services, discuss goals and care with the patient and families , answer any questions, and obtain necessary consents and legal documents
n
nnObtain medical history, current medications, and symptoms
n
nnPerform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
n
nnCoordinate the start of services (nursing, aides, equipment, medications.)
n
nnCollaborate with interdisciplinary team
n
nnMaintain accurate and timely documentation

nQualifications
nnnGraduate of an accredited nursing school with current licensure to practice in state of operation
n
nnOne year of hospice nursing experience required , 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
n
nnMust be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
n
