nCoverage area: Palatka (can support other East Coast Branches also)
nSchedule: PRN After hours on call. Must work a minimum of 2 shifts per biweekly pay period.

n nnAs a On-Call RN You will:
nnn(CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
n
nn(CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
n
nnAssess and manage urgent symptoms quickly and compassionately
n
nnAdminister medication or adjust care based on hospice protocols or provider orders
n
nnCommunicate with the physician regarding changes in condition and obtain orders as necessary
n
nnOffer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
n
nnIf a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
n

nQualifications
nnnGraduate of an accredited nursing school with current licensure to practice in state of operation
n
nnOne year nursing experience required , 3+ years preferred
n
nnExperience in hospice or a similar setting preferred
n
nnKnowledge of the hospice philosophy of care
n
nnCommitment to clinical and documentation excellence
n
nnMust be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
n
nnPossess and maintain current CPR certification in states where required.
