Job description
Sierra View Medical Center in Porterville, CA, seeks an RN Transfer Center Coordinator for Care Integration. This full-time, on-site role supports patient movement by coordinating internal and external transfers and ensuring bed availability.
The RN will triage by phone, collaborate with physicians and regional hospitals, and work with the Command Center to optimize admissions and transfers. A BSN is required; BLS; two years in acute care preferred.
