Job description
ProMedica is seeking an RN Rehab Transition Care Manager to oversee patient transitions at Flower Rehab Center. You will collaborate with physicians, nurses, therapists and families to coordinate admissions, discharges and related care across the Rehab Transition team.
The role involves extensive medical record review and active participation in team meetings and education around admission requirements and processes. This is a per diem, day-shift position based in Ohio.
