Job description
Mosaic Life Care is seeking an RHTP Navigator to join Missouri’s ToRCH Care transformation. This frontline, non-clinical role helps residents connect to clinical, behavioral, and social services by screening, referrals, and addressing access barriers across hospitals, FQHCs, and community partners.
You will document interactions in CIE systems, manage referrals, support care coordination, engage high-need residents, and contribute to hub operations with frontline insights.
