Description
Southern Tier AIDS Program, Inc. is seeking a Ryan White Peer Navigator based in our Binghamton office. This position provides confidential supportive services to persons living with HIV/AIDS. This includes accompaniment services, serving as treatment resource, serving as agency and/or client liaison with health and human service providers and providing referrals, assistance with information and appointment scheduling.
Responsibilities
Primary responsibilities of the Ryan White Peer Navigator include provision of accompaniment services to people living with HIV/AIDS, who receive services from STAP/STCC. This includes but is not limited to:
Engaging clients in the performance of: transportation services, reminder call services
Assisting with obtaining needed linkages and referrals, including: medical, legal, housing,
social, financial, transportation, food security, and other requested service
Serving as client advocate, when necessary
S upplying client data, as needed, for reporting requirements, to STAP/STCC, NYS DOH, or other funding agencies
Serves as a treatment resource for clients, case managers and community providers providing support to the doctor-patient relationship before, during and after the completion of formal skill/behavior-based
programming
Serves as an agency and/or client liaison with health and human service providers throughout the continuum of care and community. Provides referrals, assistance with information, assistance with appointment scheduling, etc.

$18.50/hour working 35-hours per week, may vary depending upon experience
Requirements
Education
High School Diploma/GED and required experience.
Experience
Excellent writing skills required.
The Peer Navigator will have lived experience and a working knowledge of issues facing persons affected by HIV & AIDS.
Organizational skills, record-keeping, time management and communication skills are essential. Computer literacy required.
Physical demands are those associated with general office duties including: sitting, standing and light lifting. This individual will travel in the community (i.e. visiting with service providers, client home visits, etc.).
