Scheduling Integrated Care CoordinatorIntention Healthcare is seeking a detail-oriented and highly organized Scheduling Coordinator to support the coordination of primary care provider (PCP) home visits. This role is responsible for scheduling patient appointments, optimizing provider routes, and ensuring seamless communication between patients, caregivers, and clinical staff. You'll be part of an organization that has demonstrated over 25 years of commitment to innovating healthcare delivery for underserved communities, with opportunities to grow alongside our rapidly expanding multispecialty medical group.Key ResponsibilitiesCoordinate and schedule in-home visits in alignment with provider availability, patient needs, and geographic considerationsConduct outbound and inbound scheduling calls while maintaining a high level of professionalism and patient-centered communicationConfirm, reschedule, and cancel appointments as necessary, ensuring minimal disruption to provider schedules and continuity of careIdentify and appropriately escalate urgent or high-priority patient needs in accordance with clinical guidelinesMonitor real-time schedule changes (e.g., delays, cancellations, provider availability) proactively adjust to maintain workflow efficiencyCollaborate with dispatch and clinical teams to resolve scheduling conflicts and ensure timely patient care deliveryPerform preliminary insurance eligibility checks and flag discrepancies for appropriate teamsPartner with billing, intake, and care coordination teams to support seamless patient onboarding and service deliveryPatient Engagement & OutreachServe as a primary point of contact for patients regarding appointment logistics, including visit windows, preparation requirements, and general inquiriesDeliver clear, accurate, and empathetic communication, particularly when working with elderly or high-risk populationsAddress and de-escalate patient concerns related to scheduling delays, cancellations, or service issues Ensure all patient interactions align with organizational standards for customer service and care experienceQualificationsHigh school diploma or equivalent required; associate or bachelor's degree in healthcare administration, business, or a related field preferred1-2 years of direct Customer Service ExperienceFamiliarity with Electronic Health Records (EHR)Prior experience coordinating in-home services, field operations, or provider scheduling strongly preferredSkills & CompetenciesBilingual: Fluent in Spanish, Mandarin, Russian (Preferred)Excellent communication and public speaking skillsAbility to work independently and travel to BronxStrong organizational and time management skillsCultural sensitivity and ability to work with diverse, underserved populationsProficiency in motivational interviewing and health coaching techniquesEqual Opportunity EmployerEssen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.
