Job description
SCAN Health Plan is seeking an Advanced Practice Clinician to deliver and coordinate primary care for members in their homes and living communities. You will conduct exams, vaccines, and tests, and develop comprehensive care plans while coordinating with the care team.
Travel up to 75% within the assigned CA area is required. California NP license or PA-C is needed, with geriatric expertise and strong collaboration skills to improve patient outcomes.
