Job description
SCAN Health Plan is seeking an Advanced Practice Clinician to deliver and coordinate primary care for SCAN members, focusing on independent, home-based care to keep seniors healthy and independent.
Responsibilities include patient exams and vaccines in senior living communities, order tests, develop care plans, and collaborate with physicians and case management. Travel 75% within the assigned region; on-call duties may apply.
