Job description
TEEMA Group seeks a dedicated RN Case Manager in San Jose, CA to own care outcomes for an elderly patient panel. You will work with PCPs and an interdisciplinary team to assess, plan, and implement care across clinic, virtual, and home settings.
You will educate patients, coordinate care across settings, and ensure timely follow-ups while maintaining compliant EHR documentation. Hybrid work model offered with field visits.
