Senior Care & Community Outreach Liaison
Are you passionate about helping seniors live safely and independently while building strong community relationships?
We are seeking a motivated, relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales, senior services, outreach, and provider relations experience.
In this position, you'll build referral partnerships, educate the community, and connect eligible seniors with life-changing care and support services.
What You'll Do
Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations
Educate referral sources, families, and community partners on program benefits
Generate qualified referrals and support enrollment growth
Conduct presentations, community events, and outreach activities
Maintain territory plans and referral pipelines
Collaborate with enrollment, clinical, and operations teams to ensure smooth participant transitions
What We're Looking For
2+ years of experience in healthcare sales, outreach, business development, or community engagement
Experience in senior care, home care, hospice, PACE
Strong networking and relationship-building skills
Excellent communication and presentation skills
Self-motivated with ability to manage a territory independently
Comfortable in assisting seniors with the process in their homes
Valid driver's license and reliable transportation
