Job description
Central California Alliance for Health seeks a Senior Care Integration Nurse (RN) for a temporary, full-time role in California. You will coordinate Enhanced Care Management, Community Supports, and transitions of care to connect Alliance members with timely, appropriate services.
The position requires CA RN licensure, an associate degree in nursing, and 7+ years in outpatient care management or related fields. Hybrid work with potential on-site responsibilities is anticipated.
