Job description
A value‑based primary care organization focused on delivering concierge‑level care to seniors without added costs is growing and expanding. Partnering with Medicare Advantage plans, the group is dedicated to improving outcomes through a patient‑first, team‑based approach and is seeking Geriatric Physicians to join their innovative care model near Montgomery, AL.
nnThis Position Offers:
nnReduced patient panel size to 400 or fewer patients, allowing for more meaningful patient interactions
nValue‑based, outcomes‑driven care model with no fee‑for‑service pressure
nCollaborative, team‑based environment including physicians, APPs, pharmacists, behavioral health, and care coordinators
nStrong administrative and clinical support, including coders and referral specialists
nMinimal call responsibilities
