Job description
CommuniCare Health in Glen Burnie, MD is seeking a Nurse Practitioner (Adult and/or Gerontology) to provide high-quality primary care in a senior-focused, high-touch model. The role supports residents across nursing facilities, assisted living, and related settings with optional telehealth.
The NP will diagnose, treat, and manage acute and chronic conditions, collaborate with an interdisciplinary team, and participate in care planning.
