Job description
Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant for the Senior Community Care program in Dutchess County. This per diem role offers meaningful work in caring for high-risk populations across nursing homes, senior housing, and assisted living settings.
Responsibilities include medical reviews, exams, and care planning in collaboration with an interdisciplinary team. A $5,000 sign-on bonus is available for external candidates.
