Job description
Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver primary care in skilled nursing and senior living facilities. You will provide direct patient care, focus on prevention and chronic disease management, and coordinate care with other clinicians to optimize outcomes.
You will conduct rounds, educate patients and families, document care in EMR, and participate in quality improvement initiatives to reduce readmissions and enhance satisfaction.
