Job description
EmpowerMe Wellness is seeking licensed therapists to deliver comprehensive therapy and wellness services to older adults within our EmpowerMe 360 program. You’ll conduct assessments, build personalized care plans, and provide evidence-based interventions as part of a collaborative team.
You will support care coordination through wellness check-ins, caregiver communication, and telehealth, while maintaining high ethical standards and contributing to program development and quality improvement.
