nWHAT WILL I BE DOING?

nCHI is looking for a full-time Senior Case Manager for our Commack location. You will be responsible for all supervisory aspects of social work services within emergency housing setting. Under the direction of the Program Manager, the Senior Case Manager will work as part of an interdisciplinary team. The Senior Case Manager will supervise the overall running of the Case Managers for their site. The Senior Case Manager will ensure quality housing services are provided effectively and in accordance with guidelines established by CHI, OTDA, Suffolk County Department of Social Services, and other funding sources as appropriate. This is accomplished by performing the following functions:

nnnProvide crisis intervention as appropriate.

n

nnSupport direct case management duties as needed.

n

nnProvide regular supervision for Case Managers.

n

nnConduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).

n

nnEnsure that the program is providing quality services and is compliant with program requirements.

n

nnPromote sound social work practices through supervision including case consultations, case file reviews, and employee performance appraisals.

n

nnEnsure the timely and correct submission of all internal and external reports in the manner required by funding agencies and/or CHI (housing logs, client contact sheets, unit inspection sheets, progress notes, authorization forms, admission and discharge sheets, census reports, etc.).

n

nnInform Program Manager of deficiencies that require additional staff training.

n

nnAct as a liaison with agencies responsible for referring eligible clients.

n

nnAdhere to proper use of company resources (cell phones, office supplies, internet, etc.).

n

nnAct as a role model to Case Management staff in accordance with agency policies and procedures.

n

nnProvide staff with ongoing training and growth opportunities.

n

nnParticipate in the hiring process, initial and annual employee evaluations, and disciplinary measures.

n

nnResponsible to oversee appropriate coverage and payroll procedures within department. Provide on-call coverage as needed.

nnnSalary: $66,300 annually. Exempt.

n

nnOpen shifts: (Monday - Friday, 8:00 a.m. - 4:00 p.m.) on-call responsibilities

n nWHAT DO I NEED?

nnnEducation: BSW required, MSW from an accredited university or related field of study preferred

n

nnExperience: 2-5 years’ experience providing management services preferred.

n

nnCertificates or Licenses: Valid driver’s license.

n

nnCommunication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish) is a plus.

n

nnComputer Skills: Basic competency in MS Windows, MS Office, and internet usage.Knowledge of Foothold Awards System a plus.

n

nnMath Skills: Basic math skills required.

n

nnPhysical Performance: Ability to tour properties, walk distances, climb stairs, etc.

n

nnReasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls.

n

nnOther Skills: Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details. Must be even-tempered, have good negotiation skills, and the ability to meet unexpected deadlines. Effective time management skills.
