Job description
Kindred Hospital Louisville is a 123-bed long-term acute care facility delivering extended recovery care with 16 Special Care Unit beds and four negative pressure rooms. The hospital is located in the Original Highlands area and collaborates with physicians to provide 24/7 clinical care.
The nurse/case management role focuses on care coordination, discharge planning, utilization review, and close teamwork to ensure safe, timely transitions and patient advocacy throughout the care continuum.
