Job description
Banner Lassen Medical Center in Susanville, CA is seeking a Case Manager to provide comprehensive care coordination for patients across units. You will conduct initial assessments, participate in rounds, and communicate discharge plans to patients and families.
The role requires RN or Social Worker credentials, BLS, and 2 years of clinical experience. CCM certification is preferred; compensation ranges per posted pay scale are provided.
