Description

The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:

(1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.

(2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required Qualifications:

Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or

PharmD in PHARMACY from an accredited School of Pharmacy.

Coursework/Training: Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.

Upon hire: Washington Pharmacist License.

Within 2 years of hire: Certification from Board of Pharmacy Specialties.

Upon hire: Board of Pharmacy Preceptor License - when eligible and where applicable.

3 years Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
