Job description
U.S. Department of Veterans Affairs is seeking a Clinical Pharmacy Specialist (CPS) to provide high-level clinical pharmacy services, directly caring for patients and acting as an advanced practice provider within assigned teams.
The CPS initiates, modifies or discontinues medication therapy and leads intensive MTM services, designing therapeutic plans and monitoring outcomes across care settings. Applicants must have a Pharm.D.
