Job description
UnitedHealth Group is seeking a qualified Advanced Practice Clinician (NP/PA) to provide acute and chronic care management to patients at home, in assisted living facilities, and SNFs in the Hamilton, Butler, and parts of Warren County, OH. This blended role combines direct patient care with transitional visits for members moving from hospital to SNF or home.
The position requires active NP or PA certification, state licensure, and the ability to travel 40 hours weekly.
